To determine the feasibility of a large-scale randomized controlled trial (RCT) of an 'EMDR for psychosis' intervention for trauma survivors with active psychotic symptoms supported by EI services, we conducted a single-blind RCT comparing 16 sessions of EMDRp + TAU versus TAU only. Method(s) EMDRp therapy and trial assessments were completed both in-person and remotely during the COVID-19 pandemic, and key feasibility outcomes (recruitment & retention, therapy attendance/ engagement, adherence to EMPRp treatment protocol, and the 'promise of efficacy' of EMDRp on relevant clinical outcomes) were examined at 6- and 12-month post-randomization assessments. Results and Conclusion(s) 60 participants (100% of the recruitment target) received TAU or EMDR + TAU. The feasibility criteria examined in this trial were fully met, and EMDRp was associated with promising signals of efficacy on a range of valuable post-treatment outcomes, including improved psychotic symptoms (PANSS), subjective recovery (QPR), post-traumatic symptoms (PCL-5;ITQ), depression (PHQ-9), anxiety (GAD-7) and general health status (EQ-5D-VAS) at the 6-month assessment. Signals of efficacy at 12-month were less pronounced, but remained robust for trauma symptoms and general health status. The findings will be discussed with relevance to future clinical trials of trauma-focused therapy in clients with early psychosis, and the provision of more tailored trauma therapies for EI service users. Eye-movement desensitization and reprocessing (EMDR) is a psychotherapy intervention designed to treat symptoms of posttraumatic stress. We conducted a systematic review to examine the viability of EMDR among first responders. The primary aim of this review was to identify studies that have trialed EMDR among first responders and evaluate its effectiveness in reducing trauma-related symptoms; a secondary aim was to identify whether EMDR has been used as an early intervention for this cohort and determine its effectiveness as such. Four databases were searched. Studies were included if they evaluated the extent to which EMDR was effective in alleviating symptoms stemming from work-related trauma exposure among first responders. The findings from each study were reported descriptively, and eight studies that evaluated the efficacy of EMDR in this population were included. There was substantial variation in how EMDR was implemented, particularly in the type, duration, frequency, and timing. The findings suggest that EMDR can alleviate symptoms of work-related trauma exposure among first responders; however, findings regarding early intervention were inconclusive, and a methodological quality assessment revealed that all studies were classified as being of either weak or medium quality. Although this review provides preliminary insights into the effectiveness of EMDR for first responders, the conclusions that can be drawn from the literature are limited, and the findings highlight several gaps in the literature. They are also subjected to continuous transitions throughout their migration trajectory (Bhugra, 2004). Although many studies have investigated the efficacy of bilateral movement in treatment of one single traumatic event (e.g. Fernandez, 2007), there is limited literature around bilateral movements interventions being used with perpetuated and multiple traumas within this cohort. This paper looks at the evidence for early intervention trauma work based on EMDR principles, the benefits of using running as a form of bilateral movement and the findings of a UASC sport trauma group with children in Kent, UK. It also gives recommendations for future research into early intervention trauma work with unaccompanied asylum-seeking children. The data showed a significant rise in the Validity of Cognition scores with a reduction in Subjective Units of Disturbance scores of those participating in the programme. This highlights the validity of this protocol and shows promising results for future enquiry. Turkey shares about 1200 km of border with Syria, Iran and Iraq. During the war in Middle East, especially in Syria, more than 3.5 million refugees from Syria moved to Turkey during the last 6 years. Until recently, Turkey has become the playground of major terrorist organizations and intelligence services of many countries. Bombings reached its peak during 2016, the last heartbreaking terrorist attack took place in Ataturk Airport in Istanbul in June 2016. It has been reported that 42 people died and more than 230 people have been injured. (www.bbc.co.uk) As EMDR Turkey Association we managed to go and reach out the victims 22 hours later. We reached out 1397 people and did psycho-educational meetings neutralizing the traumatic experience and screening the PTSD symptoms (IES-R ≥33). We did G-TEP groups and R-TEP with individuals who are in need. However, only two weeks after the bombing, the coup attempt happened and this time the same victims had a second trauma and all therapists were traumatized too. Our aim is sharing our EMDR Early Intervention Model for large scale disaster management within the experience of Ataturk Airport bombing. The steps are as follows: 1. EMDR Turkey Association has 1000 members, scattered around the country. Half of them are volunteers and members of EMDR Trauma Recovery Group. They begin sending messages how many and which days and how long they can work. In this project 40 therapists worked for 2.5 months. In 24 hours the airport began fully functioning. 2. Reaching the CEOs and the managers of the business organizations operating at the airport, discuss the intervention program and get permission to contact with the victims 3. Psychoeducational groups: Aiming normalization, introducing our psychosocial support system and screening. 4. Intervention to crisis individually and in organizational basis: Systemic interventions; leading the leaders, checking the organizations’ crisis models and give feedback in order to restrain secondary crisis. 5. Individual and group interventions: After screening, inviting who are in need of EMDR G-TEP and EMDR R-TEP for further support. 6. Creating opportunity for new therapists to learn managing the disaster, doing EMDR R-TEP and running the EMDR G-TEP. Every disaster is an opportunity for a talented young professional to become a leader in future projects. 7. What happens if a second trauma happens; a strategy for traumatized therapists 8. A small study: Is there a relationship between ACE Score and PTSD Diagnosis. It seems there is. Early intervention is defined here as intervention designed to prevent chronic emotional problems and minimize long-term deterioration in quality of life following trauma exposure. The study was designed to assess the effects of Early EMDR Intervention (EEI) on the victims of a terrorist bombing in Gungoren, Istanbul. Subjects were the victims of a terrorist bombing in Gungoren, Istanbul. The participants were selected from a pool of children and adults, who lived in Gungoren and scored high on the Turkish version of 'Impact of Event Scale' (IES) and PTSD Symptom Checklist. The subjects were contacted 2 days after the bomb attack so that we were able to measure the event impact right after the traumatic experience, which will help us to demonstrate how EMDR affects the impact of the event more accurately. Eye Movement Desensitization (EMD) as an EEI technique was used to treat the child participants, whereas Recent Traumatic Event Protocol (R-TEP) which incorporates the EMD and Recent Event (RE) protocols, was received by the adult participants The therapists (EMDR certified therapists, who were receiving supervision) met with the participants weekly to work only on the trauma of the bombing and participants completed impact of Event Scale prior to each session. The number of the sessions was restricted to the completion of EMD and R-TEP. The study is completed with a three month follow-up. Offering structured psychological interventions such as eye movement desensitization and reprocessing (EMDR) in theater has a number of potential advantages. This single-case study describes how the EMDR recent event protocol (Shapiro, 1995) was used in theater with a 27-year-old active-duty U.K. soldier who was experiencing an acute stress reaction after treating a land mine casualty. The intervention took place 2 weeks posttrauma with four sessions conducted on consecutive days, resulting in a positive outcome, with the soldier able to return immediately to frontline duties. Treatment response was assessed with administration of four standardized measures at pretreatment, posttreatment, and 18-month follow-up. Treatment effects remained at 18-month follow-up. The challenges of conducting EMDR in operational theaters and clinical implications are explored. EMDR is now widely used to treat UK military personnel suffering from combat related PTSD. This presentation will consider extending the use of EMDR as an early intervention during frontline operations. Research suggests the importance of early detection in PTSD (Lee et al 2005) and the benefits of early intervention (North 2001). Current practice is that UK personnel who are having significant difficulties in functioning after exposure to trauma are sent back to the UK for treatment after a brief period of watchful waiting. This is can be detrimental, both to the unit and individual (Shepard 2000, Solomon et al 2005). EMDR offers a potential solution to this problem. A case study will be presented of a soldier who was suffering significant post trauma symptoms whilst serving in Afghanistan. Through the successful use of EMDR in theatre just 2 weeks after the trigger event, he was able to resume his normal duties and hence avoid the negative effects of being returned to the UK for treatment. The use of EMDR as a frontline treatment has far reaching implications but currently there is little evidence for its use as an early intervention with military personnel (Russell 2006). Future research to evaluate this novel application of EMDR is planned. This presentation will introduce participants to the challenges of providing psychological treatment during military operations and the potential value of EMDR in this setting.
