This research addressed psychotic symptoms as presenting earlier traumas. Few studies have utilized EMDR to address the occurrence of a manic episode in those diagnosed with Bipolar 1 Disorder. Manic episodes are characterized by excessive involvement in activities that have potential for painful consequences. These include unrestrained buying sprees, sexual indiscretions or unwise business investments. The aftermath of these behaviors can produce feelings of embarrassment, shame, inadequacy and confusion. When the events of a manic episode are traumatic for clients, EMDR should be considered as a treatment option. If traumatic memories of a manic episode are not addressed in treatment, persons experiencing Bipolar 1 Disorder may not fully recover or return t premorbid functioning. One case study is presented in which a client with Bipolar 1 Disorder who experienced a manic episode as a traumatic event was treated with EMDR. After treatment with EMDR, the client experienced improvement in quality of life, lowered level of disturbance of the original event as well as overall acceptance and increased management of the disorder. Further research is needed to determine a protocol for treating person with Bipolar 1 Disorder utilizing EMDR. In Chinese culture which is collectivism-oriented, people deal with grief not only in a society context, but also have a lot of connection with the bereaved ones. We found that it is hard to help the clients to be stabilized and work with EMDR on their grief before you help them to find a “Safe place” to settle their bereaved ones down. Hence, we use a modified “safe place” technique to help the clients to create a “heaven” for their loved ones, a “place” they can visit to say “hello” again to their bereaved ones. Our presentation will how we did that in a case series of adults and children by using imaginary ways and artistic ways. Using EMDR as standard protocol was effective to remove the symptoms and got the SUDs down. Both treatments showed very large effect sizes on the post-traumatic symptom outcomes, and a modest effect size on the behavior outcome, all statistically significant. A non-significant trend on self-reported post-traumatic stress symptoms favored EMDR over CBT.Treatment efficiency was calculated by dividing effect size by number of sessions; EMDR was significantly more efficient. Limitations include small N, single therapist for each treatment condition, and lack of long-term followup. These findings suggest that both CBT and EMDR can help girls to recover from the effects of sexual abuse, and that structured trauma treatments can be applied to children in other cultures. Despite the complexity and variety of presenting issues, there is a set of more or less standardized steps to follow to ensure safety as well as appropriate follow-up. In this paper I present a case to illustrate how I have integrated EMDR. This approach to teen suicidality uses several elements of the motivation - anger - trauma therapy (MATT) approach for teens with conduct disorder, which has been described in detail elsewhere (Greenwald 1998, 1999, in press). In addition to the standard crisis interventions - letting the client talk out the problem, contracting for safety, implementing supports and restrictions as needed, and arranging follow-up - I have been using EMDR in various ways to enhance present safety as well as subsequent resiliency. The research team has found that the combination of expressive arts techniques and EMDR treatment can be highly effective in overcoming these impediments. This poster will briefly review basic knowledge and practice concerning the major eating disorders and their etiologies. We will then describe our therapeutic process that combines expressive arts with EMDR. This presentation reviews all EMDR randomized controlled trial studies conducted with children from 1998 to 2008 and summarizes outcomes, methodological approaches and limitations, as well as recommendations for future research. Research indicates that EMDR may be a promising treatment for children experiencing PTSD symptoms, or other residual traumatic effects from distressing occurrences. The center’s advocacy staffed administered treatment for the SA group and the P/SA group. In addition to the SA treatment received by individuals in the P/SA group, there were also six licensed therapists who provided a manualized treatment incorporating CBT and EMDR. Symptoms of PTSD, depression, CSE, self-esteem, and coping skills were assessed.Additionally, saliva sample measurements were taken to examine the effect of treatment on the regulation of salivary cortisol. Results indicate that participants in the P/SA treatment condition experienced a significantly greater reduction in intrusive symptomatology and overall level of PTSD symptom severity, as well as a greater increase in CSE over time than those individuals in the SA condition. Moreover, changes in CSE were related to changes in psychological symptoms in individuals in the P/SA group from pretreatment to posttreatment and at follow-up.Almost no linear relationship between changes in CSE and other psychological variables in the SA group emerged.A minimal impact of treatment on coping skills was observed. There was a move towards a greater regulation of salivary cortisol at posttreatment in the P/SA group. Since the touchstone event is pre-verbal, it is difficult to identify, but crucial in the resolution of later traumas resistant to full EMDR processing (Those who remain stuck at a greater than 0 SUDs). The antedotal experience is that this model can activate the material more fully and facilitate more thorough competion of phases 4-8. "Once upon a Time" contains every element of the EMDR Protocol in the prescribed order, Incident, Image, NC, PC, VOC, Emotion, SUD, Body sensation. This experimental model is not a substitute for standard EMDR. It facilitates access tohese preverbal traumas and the resultant cognitions which may have formed around them. It allows for a return to the standard EMDR protocol after this early material has been effectively targeted and reprocessed. "Once Upon a Time" model allows for fuller connection with early material. History is collected through antedotal information from third party informants and family photographs and is used to create a metaphor; this technique can access the multiple modalities of pre-verbal experience previously intellectualized. Phase three begins with a short continuation of Phase 1 using an interview format to review and briefly discuss the various elements of the troubling material. A "sentence completion" format is used to obtain the TICES elements. Those spontaneous answers form the script for a "Once Upon a Time" (Crow, 2004, EMDRIA Montreal, Canada), a deviation from Phase 4 of the standard protocol. Pertinent examples of the application of this model will be discussed and demonstrated with video taped excerpts of actual clients. Video taped client reports of the long term effect of the shifts resulting from the "Once Upon a Time" experience will provide validation that this technique enables retur to the standard EMDR protocol and full processing of previously incompletely processed material. Participants will create their own "Once Upon a Time" script from a video example of client history as an experiential introduction to the intricacies of this model. Evidence indicates that this technique is effective on a "consultant" basis, where the "Once Upon a Time" can be conducted by a separate therapist skilled in the technique, and returned to their regular EMDR therapist to complete this treatment. A total of 9 subjects agreed to be interviewed regarding their EMDR practice and integration issues. This qualititative data was content analyzed. No differences were found between both groups on Biodata factors, years of experience as a Therapist or years using EMDR which gives further confidence in the results when comparing both cohorts. Results: Respondents endorsed two types of explanation as to why they believed EMDR works (ie) EMDR facilitates communication between the Limbic system and Frontal Lobes (77%) and Adaptive Information Processing (73%). This suggests that Therapists are not just following Shapiro’s AIP model slavishly but are rather making up their own minds. There were no significant differences between Analytic, CBT, Integrative and Humanistic Therapists in terms of explanatory mechanisms endorsed about EMDR’s “active ingredient”. CBT Therapists found it easier to incorporate EMDR into their clinical practice than Analytic or Humanist Therapists. This finding was supported by the results of the qualitative interview data. Indeed, up to 40% of Therapists sampled experienced difficulties in integrating EMDR into their clinical practice, post training. The types of difficulties identified included: Differences in theoretical beliefs, more active style of EMDR, structure of EMDR Protocol, Therapist confidence issues, Organizational issues and hostility from clinical colleagues, bullying, lack of supervision and support post training. Conclusions: The results of this study confirm findings from earlier studies regarding Therapists’ beliefs about EMDR but also extend those finding internationally so that previous findings can now be accepted as universal given that this present study contained respondents from three other continents other than North America. This study also found that up to 40% of Therapists trained in EMDR experienced significant difficulties in integrating EMDR into their clinical practice post-training. Analytic and Humanist Therapists reported the biggest difficulties which included conflict with the Therapists’ original theoretical model, the EMDR Protocol structure itself, Therapist confidence and lack of supervision and support, Organizational and Management issues of opposition to EMDR and Therapist Bullying. The implications for EMDR training are discussed and communicated to relevant EMDR Training Organizations. A second aim of my study is to examine how EMDR trained Therapists’ explanations relate to their original clinical training. The Research Question is: Do CBT trained Therapists differ in the explanations they give about the therapeutic mechanisms of EMDR from, say, analytically oriented Therapists? Do Humanistic Therapists explain how EMDR works from existentially oriented Therapists? A third aim of my study is to examine how Therapists from different backgrounds incorporate EMDR into their clinical practice. The Research Question here is: Do Therapists from some backgrounds such as CBT find it easier to incorporate EMDR into their everyday clinical work than other types of Therapists? Research Methodology: I 24 shall use a combination of quantitative and qualitative methods in this research project. The quantitative method will be a short 2 page anonymous questionnaire which will be given out at the Conference for delegates to complete and return to the stand. I would hope to have at least 50 such Questionnaires completed by consenting delegates to the Conference. The qualitative methods will be a number of focus groups (maximum number of 12 people in each group) which I hope to run at the end of each day of the Conference. I plan to have 3-4 such focus groups from which the data will be gathered. The other qualitative method will be 1:1 semi-structured interviews either face to face at the Conference or by telephone (post conference) with consenting delegates attending the Conference. Altogether, I would hope to have a total of about 50 participants in the qualitative data gathering methods. Our clinical team members are EMDR psychotherapists trained by EMDRIA approved trainers and the team continues on the EMDR path by completed advanced trainings. As we anchored ourselves in EMDR psychotherapy, we developed our milieu to accompany our girls in their healing process. This led us to understand how the milieu can directly support all eight phases with a strong emphasis on the preparation phase. To do this we created multiple adjunctive therapies to feed the resourcing and regulatory capacities of our girls. The purpose of their exploratory clinical pilot project was to measure the impact of this on the functioning of our girls. The clinical project was developed to guide us in being able to establish a research agenda. However, often these behaviours are in response to deep-seated memories linked to traumas, which with the help of EMDR could safely be processed once the impulsive and risky behaviours are controlled. Mindfulness has been utilised by the Author as a stabilisation method of reducing dissociation in clients, prior to trauma processing (CEP conference – Darker-Smith, 2005). More recently, the author has discovered that the application of mindfulness and imagery techniques work more effectively for clients with tendencies for self-harm, compared to alternative behavioural techniques designed to distract from or substitute for impulsive desires to self-harm (e.g.., the use of ice cubes or elastic bans, to create a distraction from the impulse). Two groups were studied in the process of treating co-morbid symptoms for alternative conditions with EMDR, ranging from eating disorders, anxiety disorders, and trauma, prior to EMDR processing. For clinical reasons, clients with depression, personality disorders and other Axis 2 disorders were not included in this study due to contraindications in current research relating to Mindfulness. Participants self-harming behaviours related to superficial cutting, punching, and burning. Group 1 consisted of six clients who were offered alternative behavioural techniques (e.g., elastic bands or ice cubes) to distract or substitute for the desire for self-harm. Group 2 consisted of eight clients who were offered mindfulness techniques, including imagery meditations to distract or substitute for the desire to self harm. The groups were distributed as evenly as possible and no major emphasis was placed on the treatment of self-harming behaviours, instead being placed on the major problems (anxiety, eating disorder or trauma). The Group (1)[consisted of 6 persons:(3 with Anxiety, 3 with Eating Disorders, 1 with Trauma)] who were offered suitable behavioural techniques utilised them effectively when their distress levels were mild (between 1-4 on a 0-8 behavioural scale), however, reverted back t self harming behaviours (e.g., cutting, burning, pinching) when distress levels reached 5 or higher. The Group (2)[consisted of 8 persons: (3 with Anxiety, 4 with Eating Disorders, 1 with Trauma)] who were offered aspects of Mindfulness training to facilitate tolerance of distressing emotions and being aware of the active moment did not tend (on average) to revert back to self-harming behaviours, choosing instead to utilise mindfulness methods (such as 3-minute breathing space). Conclusion: Mindfulness is more effective as impulse control for self-harming behaviours than behavioural alternative strategies and can be utilised as a form of stabilisation in combination with controlling impulsive behaviours, prior to EMDR. Although health services and facilities had been re-established and some NGOs started delivering psychosocial and mental health services, only a few of them focussed on effective trauma therapy while the number of survivors who need the treatment are enormous. Tsunami survivors in this region are more likely to suffer from complex PTSD because of years of armed-conflict had been going on in this region. A survey conducted by Crisis Centre of the Faculty of Psychology University of Indonesia in collaboration with Terre des Hommes Germany showed high incidents of various psychological disorders amongst child survivors.. The program of trauma therapy and EMDR organized by Indonesian Psychological Association and TdH Germany funded by BMZ Germany since 2006 has treated a good number of tsunami survivors with severe PTSD. Hyperarousal, flashbacks and bad dreams, avoidance, and somatisation are common. There has been no indication whether there has been a natural process of recovery among those who were not treated. Starting in February 2008 a controlled study on the effectiveness of EMDR is conducted with 30 tsunami survivors with PTSD that will be randomized into two groups of 15. The treatment group will be given EMDR therapy until April 2008 and waitlist group will be given EMDR in May 2008.
