IG-11 心理的外傷体験にEMDRを用いて改善した二例(パニック) EMDR experience psychological trauma for improved two cases (panic)

〔は じめ に 〕 EMDR （眼 球 運 動に よ る 脱 感 作 と 再 処 理 ） は PTSD （外 傷 後 ス トレ ス 障 害 ） の 治 療 法 と し て 開 発 され た 認 知 行動 療 法 技 法 で あ る が 、今 回 、心 理 的 外傷 体 験 に よ り発 症 し た 大 うつ 病 と 社会 恐 怖 の 症 例 に EMDR を 適 用 し改 善 を 見 た の で 報 告す る 。 〔症 例 1 ： 1 主 訴 ； 離婚 に 向 け て 心 の 整 理 を した い 。 31 歳 、女 性 、会 社 員 。 平 成 10年 1月 、夫 の 浮 気 に 気 付 い て か ら、抑 うっ 気 分 、孤 独 感 な ど が 出現 し た ため心 療 内科 受 診 し、抗 うつ 剤治 療 に て や や軽 快 ， 同 年 4 月 、主 治 医 の 紹 介 に て 当 科 外 来 受 診 。治療 経 過 ：夫の 浮 気の 他に も幼 少期に 友 人 か ら拒 絶 され る な ど の 外 傷 体 験 が 認 め られ 、他者 か ら の 拒 絶場 面 に 対 し て EMDR を 適用 した 。 　「夫か ら 浮気 を告 げ られ た 」 場 面 な ど を 夕 一ゲ ッ トと して 8 回 の EMDR セ ッ シ ョ ン を 実 施 し た と こ ろ 、次第 に 夫 の 心 変 わ りを 受 容 で き る よ う に な り、抑 うつ 気 分 も軽 快 した 。 〔症 例 2 ｝ 主 訴 ：対 人 緊 張 。 16歳 、高 校 1 年生 、女 性 。平 成 6 年 （中学 2 年 ） 、同級 生 の 男 子 か らい じ め られ 、翌 年 に は 同 級 生 の 女 壬か ら無 視 さ れ 、抑 う つ 気 分 、不 眠 、対 人 緊 張 な どが 出 現 。平 成 8 年 の 高 校 進 学 後 、抑 うつ 気 分 は 軽 快 した が 対 人 緊 張 は 不 変 の た め 、高校 の 養 護教 諭の 紹 介 で 平 成 9 年 3月 高校 の カ ウ ン セ ラ ー室 へ 来 談。 治 療 経 過 ： 「友 だ ち に 話 し掛 ける 」 な ど の 場 面 を タ ーゲ ッ トと して 4 回 の EMDR セ ッ シ ョ ン を 実施 し 、平 行 して 「映画 館 に 行 く」 な ど の 課 題 を 用 い た in　vivo 　expc 】sure を 実施 し た と こ ろ 対人 緊 張 は 改善 した。 〔結 論 〕 心 理 的 外 傷 体 験 を 契 機 と して 発 症 し た 気分 障 害 、不 安 障 害 の 治 療 に EMDR が 有 効 で あ る こ とが 示 唆 さ れ た.
