Eye movement and reprocessing from the perspective of a client: A case study of EMDR in Pakistan

BACKGROUND: Eye Movement Desensitization and Reprocessing (EMDR) has demonstrated efficacy in the treatment of recurrent depression secondary to childhood sexual abuse. OBJECTIVES: This case examines the effectiveness of EMDR and its potential pitfalls and difficulties as experienced by a client. METHOD: Our client was a healthcare provider with a history of recurrent depression and childhood sexual abuse. Failing to respond to antidepressant therapy, she was enrolled in EMDR to address the underlying traumatic memories of the abuse. Client feedback at the end of each session was recorded, and a detailed interview was conducted at the end of therapy to explore her experiences throughout the EMDR process. RESULTS: The main concern expressed was the focus of the therapy being on the past trauma instead of the present difficulties faced due to depressive symptomatology. This was addressed by providing a rationale for targeting and processing underlying trauma which would indirectly improve depressive symptoms. Other concerns included that in establishing safe place, there should be flexibility to include imaginary places. The option of tapping instead of eye movements should be given at the beginning of the therapy. The client also found it difficult to verbalize the negative cognitions and felt that providing a list of cognitions would be helpful. CONCLUSIONS: The findings appear to provide practical information for EMDR practitioners so they can be mindful of the concerns of the clients in their respective practices and facilitate better treatment outcomes.
