Göz hareketleriyle duyarsızlaştırma ve yeniden işleme (EMDR), travmatik anıların işlenmesi yoluyla var olan problemin çözümlenmesini hedefleyen bir psikoterapi yöntemidir. Bu çalışmada 23 yaşında kadın bir danışanın 16 yaşında yaşadığı tecavüz anısının EMDR ile çalışılması merkeze alınmıştır. Danışanın psikolojik yardım alma nedeni, romantik ilişkilerden kaçınmasıdır. Danışanı EMDR’a hazırlamak için öncelikle psikoeğitim verilmiştir. Psikoeğitimin amacı danışana EMDR ve çalışma sistemi hakkında bilgi vermek, işlemleme sürecinin ne olduğunu ve nasıl ilerlediğini detaylarıyla aktarmaktır. Bu aşamadan sonra stabilizasyon çalışmaları yapılmıştır. Stabilizasyon çalışmalarının amacı danışanın duygu regülasyonunu sağlamasını kolaylaştırmaktır. Özellikle travmatik anılar söz konusu olduğunda psikoeğitim ve stabilizasyon çalışmaları psikoterapinin önemli bir bileşeni olmaktadır. Terapi planı için EMDR standart protokol kullanılmıştır. Bu nedenle danışan ile geriye akış tekniği kullanılarak terapi planı oluşturulmuştur. Geriye akış tekniği için danışan ile birlikte belirlenen negatif kognisyon “yetersizim” olduğundan danışanın “yetersizim” inancına dair anıları terapi planına dahil edilmiştir. Danışanın ilk çocukluk yıllarına dair getirdiği anı sayısı bu sırada kısıtlı kalmıştır. Standart protokol ile çalışma sürecinde ilk anı çalışılması önerildiğinden danışan ile ilk anısı çalışılmıştır. Haftada bir yapılan ortalama 60 dakikalık 3 seans sonunda danışanın ilk anısı tamamlanmıştır. Bu sırada danışan, ilk çocukluk yıllarından anılar hatırlamaya başlamıştır. Danışan ile terapi planı yeni anılar geldikçe güncellenmiştir. İlk anının tamamlanmasının ardından en kötü anısına geçilerek tecavüz anısı çalışılmaya başlanmıştır. 4 seans sonunda anı çalışmasında istenilen VoC ve SUD değerlerine ulaşılmıştır. Toplam 7 seans ve iki anı çalışmasından sonra danışanın romantik ilişkilerinde hissettiği rahatsızlık hissi de azalmaya başlamıştır.
Desensitization and Reprocessing Through Eye Movements (EMDR) is a psychotherapy method aimed at resolving existing problems by processing traumatic memories. This study focuses on working with the rape memory of a 23-year-old female client, which occurred when she was 16 years old, using EMDR. The reason the client sought psychological help was her tendency to avoid romantic relationships. To prepare the client for EMDR, psychoeducation was first provided. The purpose of psychoeducation was to inform the client about EMDR and its working system, and to explain in detail what the processing phase is and how it progresses. After this stage, stabilization exercises were conducted. The aim of these exercises was to help the client regulate her emotions more effectively. Especially when traumatic memories are involved, psychoeducation and stabilization are essential components of psychotherapy. The therapy plan was based on the standard EMDR protocol. Therefore, the plan was created using the "float back" technique. Since the negative cognition identified with the client for the float back technique was “I am inadequate,” memories related to this belief were included in the therapy plan. At this stage, the number of memories the client could recall from early childhood was limited. As the standard protocol recommends starting with the earliest memory, the first memory was processed with the client. After three weekly sessions of approximately 60 minutes each, the client completed the work on her first memory. During this time, she began to recall additional memories from early childhood. The therapy plan was updated as new memories emerged. Once the first memory was processed, the therapy moved on to the worst memory—the rape experience. After four sessions, the desired VoC (Validity of Cognition) and SUD (Subjective Units of Distress) scores were achieved in processing the memory. Following a total of seven sessions and work on two memories, the client began to experience a reduction in the discomfort she felt in romantic relationships.
