Depuis sa découverte fortuite en 1987, la thérapie EMDR connait un intérêt scientifique et public grandissant, devenant le traitement le plus étudié pour la prise en charge du TSPT. Elle est aujourd'hui recommandée pour la prise en charge de ce trouble par un grand nombre d'autorités sanitaires internationales, dont l'Organisation Mondiale de la Santé. Si son efficacité est communément admise, ses mécanismes d'action restent méconnus et ont fait l'objet de nombreuses théories. Après avoir abordé le cas de l'EMDR, son histoire, sa description et sa validité scientifique, nous avons détaillé plusieurs de ces hypothèses, à savoir les similitudes avec les mouvements oculaires du sommeil paradoxal, l'abord neuroscientifique enrichi de l'évolution des techniques d'imagerie médicale, l'adaptation aux théories de la reconsolidation de la mémoire et de la mémoire du travail; nous discutons également de son aspect intégratif.

Since its fortuitous discovery in 1987, EMDR therapy has become increasingly scientific and publicly important, becoming the most studied treatment for the management of PTSD. It is now recommended for the management of this disorder by a large number of international health authorities, including the World Health Organization. While its effectiveness is widely accepted, its mechanisms of action remain unknown and have been the subject of many theories. After discussing the case of EMDR, its history, its description and its scientific validity, we detailed several of these hypotheses, namely the similarities with the eye movements of REM sleep, the first neuroscientific enriched by the evolution of medical imaging techniques, adapting to theories of reconsolidation of memory and memory of work; we also discuss its integrative aspect.
