Use of therapist rotation model for eye movement desensitization and reprocessing (EMDR) in patient with atopic dermatitis

Background and aims
Atopic dermatitis (AD) is a chronic inflammatory pruritic disease characterized by erythematous, dry, egzematous lesions. Up to 70% of patients may experience stressful life events before onset of AD and close relationship between psychogenic stress and AD has been demonstrated. In this study we aimed to validate efficacy of Eye Movement Desensitization and Reprocessing (EMDR) in addition to dermatologic therapy, for the relief of egzema and pruritus which was claimed to be triggered by psychological stress. We also used therapist rotation model to validate its potential therapeutic role.

Methods
We used therapist rotation model for EMDR in-patient with AD. Our therapist rotation model was based on Van Minnen et al. We compared test results including depression, anxiety, and AD disease severity index (EASI: Egzema Area and Severity Index, VAS: Visual Analogue Scale for itching) before and after EMDR session. We performed one EMDR session per week for 8 weeks in 90 minutes with four therapists.

Results
Our patients’ complaints and depression anxiety scores decreased after EMDR therapy. VAS score dropped down from 10 to 2 however EASI score did not change (14.4 vs. 14.4). BDI, BAI, IOE, and SUD scores decreased 90%, 88% and 100%, respectively.

Conclusion
EMDR may be useful for psychiatric comorbidity, psychogenic component and itching complaint of AD. Therapist rotation model may be rational, effective and tolerabl for these patients. Learning Objectives: EMDR and specific therapist rotation model may be useful for dermatologic patients with psychogenic stress.
